Skilled Nursing -Our Registered Nurses will conduct initial, and ongoing, comprehensive assessments of the patient’s needs, including, Outcome and Assessment Information Set (OASIS) assessments. We provide patient, family and primary caregiver education specific to each patients needs which may include wound care, IV Therapy Injection, Skilled observation and Assessment, Chronic Disease Management: COPD, CHF, DM, HTN, Pain Management, Post-Surgical Care, Post-Stroke Care, Fall Injury Prevention and Education, Medication Teaching and Management, and Diet education.
Physical Therapy - Our Physical Therapist will conduct the rehabilitative assessment, evaluation and treatment as well as patient, family and caregiver education and training. They will provide training on transfer, gait, and mobility skills, and durable medical equipment assessment. They will instruct patients regarding care and use of wheelchairs, braces, crutches, canes and prosthetic, as well as orthotic devices and develop an individualized home exercise program designed to improve range of motion, strength, functional mobility and balance and fall prevention. They will teach, supervise, and counsel the patient and family regarding the physical therapy program.
Occupational Therapy - Our Occupational Therapist will assess and evaluate the patient's home situation, to assist the Patient, family and primary caregiver in the development, and implementation, of the interdisciplinary patient care plan. They will guide, and instruct, the patient in prescribed therapeutic self-care and creative activities that are directed toward improving independence and physical and mental functioning. They will instruct the patient in care, and use, of wheelchairs, braces, splints, and prosthetic and orthotic devices. They will teach, supervise, and counsel the patient, and family, regarding the Occupational Therapy program and other related problems the patient may have at home.
Speech Therapy -Our Speech Therapist will provide skilled speech pathology service, on an intermittent basis, to patients, in their homes, evaluate the patient's level of functioning and recommend mechanisms to enhance the patient's hearing and swallowing ability. They will teach, supervise, and counsel the patient, and family, in the Speech Therapy program and other related issues the patient may have at home.
Medical Social Services - Our Medical Social Workers will evaluate, and assess, the social and emotional factors related to the patient's health problem, as a means of estimating the patient's capacity and potential to cope with the problems of daily living. They will provide supportive casework for patients, and their families, geared toward the acceptance of their health problems. They will provide information concerning potential secondary effects of an illness, especially emotional components. They will also act as an intermediary between patients, and other community agencies, and assist with nursing home placement if necessary. They will assist our patients, and their families, to locate and utilize community resources.
Home Health Aide - Our Home Health Aides will assist patients on an intermittent basis with their personal care needs, provide support with daily living activities, light housekeeping, bathing, grooming, dressing, and meal preparation.
